    Care & Support Workers:

    This care service involves a programme of personal care and household care management that is personalised for each service user in the form of a care plan. Care duties will therefore include assisting the service user with a variety of duties, and in doing so the carer will observe and respect the service user’s dignity, privacy and independence as far as is practical.

    Care Co-ordinator:

    A care coordinator job is often the single most important role involved in the care of any individual patient. Supervising interdisciplinary care by bringing together the different specialists whose help the patient may need, the coordinator is also responsible for monitoring and evaluating the care delivered.

    Live in Carer:

    Live-in Care Workers are employed to work with clients on a 24 hour basis to provide care and support to the client/s as detailed specifically in the care plan provided for each client. All clients are individuals who will have received a person-centred holistic assessment of their needs which forms the basis of their care plans. The role of the care worker is to meet their clients needs and to ensure they are supported as detailed in their care plan ensuring at all times that their privacy, dignity and independence are maintained. This will include not only assisting with the client’s personal care needs but also their social welfare. The role may include shopping, cooking and will include light housekeeping duties. Care packages will vary from providing companionship care through to providing palliative end of life care and care worker pay rates will vary accordingly. Care worker must work as directed by the Registered Manager and detailed in the individual care plan. The aim of 24 hour live-in care is to enable clients to remain in their homes with as much independence as possible, carrying out the tasks in a professional, discreet and caring manner whilst respecting the clients’ dignity, privacy, property and confidentiality.

    Day to day duties:

    Given the wide remit of the job, a care coordinator’s regular duties can be quite varied. Each piece of work they do begins with a meeting with the patient, kinship carers and other family members to discuss their needs, the services available to them and the help they want.

     

    A care plan is drawn up on this basis and the coordinator is then responsible for contacting other care departments or agencies to ensure that services are delivered appropriately.

    Daily tasks generally
    include the following:

    Developing care plans and rearranging them as necessary when difficulties arise.
    Visiting patients, checking on the care they’ve received and documenting it accordingly.
    Working with the care team to evaluate interventions and identify where and when further ones will be required.
    Reading, attending workshops and liaising with professional bodies to stay abreast of developments in the field.

     

    A coordinator needs to understand the importance of listening to patients and respecting their rights as well as adhering to relevant laws and to the standards of care. Given the team supervision aspect of the job, the coordinator must also be responsible for team safety.
